Before registration will be complete, all students entering LVISD for the first time must have:

Up-to-Date Immunization Records

Birth Certificate

Social Security Cards (a state ID may also be used)

Proof of Residency (Utility Bill; lease/purchase agreement)

Last report card or transcript of grades.

TAKS results if available

Accompanying Parent or Guardian with a photo ID

Custodial paperwork (if applicable)

Once we receive the above information, we can contact your former school for student records, etc.

Immunization Requirements 2009-2010 School Year

3 year olds and 4 year olds (Pre-K)

4 doses of DPT, DTaP, DT 3 doses of Polio 1 dose of MMR on/after 1st birthday 1 dose of HIB on/after 15 months OR 3 doses with the 3rd dose given on/after 1st birthday, and at least 2 months since dose #2 1 dose of PCV7 on or after 24 months of age or 4 doses of PCV7 with one given after 1st birthday 3 doses of Hepatitis B 1 dose of Varicella on/after 1st birthday (if the child has NOT had chickenpox) 2 doses of Hepatitis A (Does not exclude student from enrolling unless there has been @ least 18 mos. between immunizations.)

Kindergarten

5 doses of DPT, DTaP, DT OR 4 doses if one dose is given on/after the 4th birthday 4 doses of Polio or 3 doses of Polio are required IF 3rd dose was received on/after 4th birthday 2 doses of MMR with the first dose on/after the 1st birthday 3 doses of Hepatitis B 2 doses of Varicella on/after 1st birthday (if the child has NOT had chickenpox) 2 doses of Hepatitis A with the first dose on or after 1st birthday

First Grade

5 doses of DPT, DTaP, DT OR 4 doses if one dose is given on/after the 4th birthday 4 doses of Polio or 3 doses of Polio are required IF 3rd dose was received on/after 4th birthday 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday 3 doses of Hepatitis B 1 dose of Varicella on/after 1st birthday (if the child has NOT had chickenpox)

Second Grade

3 doses of DPT, DTaP, DT, Td w/one dose given on/after 4th birthday; (If less than 7 years old for DPT, DTaP, DT, Td see First Grade) 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday 3 doses of Hepatitis B 1 dose of Varicella on/after 1st birthday (if the child has NOT had chickenpox)

Third Grade – Fifth Grade

3 doses of DPT, DTaP, DT, Td with one dose given on/after the 4th birthday 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday 3 doses of Hepatitis B 1 dose of Varicella on/after 1st birthday (if the child has NOT had chickenpox)

Sixth Grade

3 doses of DPT, DTaP, DT, Td with one dose given on/after the 4th birthday 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday 3 doses of Hepatitis B 1 dose of Varicella on/after 1st birthday (if the child has NOT had chickenpox)

Seventh Grade

3 doses of DPT, DTaP, DT, Td with one dose given on/after the 4th birthday 1 dose of Tdap/Td booster if 5 years since their last dose of tetanus-containing vaccine 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday 3 doses of Hepatitis B 2 doses of Varicella on/after 1st birthday (if the child has NOT had chickenpox) 1 dose of Meningococcal

Eighth - Twelfth Grade

3 doses of DPT, DTaP, DT, Td with one dose given on/after the 4th birthday 1 Tdap/Td booster if 10 years since their last dose of tetanus-containing vaccine 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday 3 doses of Hepatitis B 1 dose of Varicella on/after 1st birthday (if the child has NOT had chickenpox) Two doses are required if the vaccine was given at age 13 or older.

Notes
• All vaccine doses administered less than or equal to four days before the minimum interval or age shall be counted as valid.
• Does not need a dose of Polio after the 4th birthday if 4 doses of clearly documented combination of IPV and OPV were given before 4 years of age.
• HIB and PCV7 are not recommended for children 5 years of age or older.
• Polio is not required for age 18 and above.
