New Student Registration
Before registration will be complete, all students entering LVISD for the first time must have:
  • Up-to-Date Immunization Records
  • Birth Certificate
  • Social Security Cards (a state ID may also be used)
  • Proof of Residency (Utility Bill; lease/purchase agreement)
  • Last report card or transcript of grades
  • TAKS results if available
  • Accompanying Parent or Guardian with a photo ID
  • Custodial paperwork (if applicable)
Once we receive the above information, we can contact your former school for student records, etc.
Immunization Requirements 2012-2013 School Year
Requisitos de inmunizacion - 12-13
Kindergarten-Second Grade
  • 5 doses of DPT, DTaP, DT OR 4 doses if one dose is given on/after the 4th birthday
  • 4 doses of Polio or 3 doses of Polio are required IF 3rd dose was received on/after 4th birthday
  • 2 doses of MMR with the first dose on/after the 1st birthday
  • 3 doses of Hepatitis B
  • 2 doses of Varicella on/after 1st birthday (if the child has NOT had chickenpox)
  • 2 doses of Hepatitis A with the first dose on or after 1st birthday
Third Grade-Sixth Grade
  • 5 doses of DPT, DTaP, DT OR 4 doses if one dose is given on/after the 4th birthday
  • 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday
  • 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday
  • 3 doses of Hepatitis B
Seventh Grade
  • 3 doses of primary series and 1 DTaP/Td booster within last 5 years
  • 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday
  • 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday
  • 3 doses of Hepatitis B
  • 2 doses of Varicella on/after 1st birthday
Eighth Grade-Ninth Grade
  • 3 doses of primary series and 1 DTaP/Td booster within last 10 years
  • 1 dose of Tdap/Td booster if 10 years since their last dose of tetanus-containing vaccine
  • 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday
  • 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday
  • 3 doses of Hepatitis B
  • 2 doses of Varicella on/after 1st birthday (if the child has NOT had chickenpox)
  • 1 dose of Meningococcal
Ninth-Twelfth Grade
  • 3 doses of primary series and 1 DTaP/Td booster within last 10 years
  • 4 doses of Polio OR 3 doses if one dose is given on/after 4th birthday
  • 2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the 1st birthday
  • 3 doses of Hepatitis B
  • 1 dose of Varicella on/after 1st birthday (if the child has NOT had chickenpox) Two doses are required if the first dose was given at age 13 or older.
